New Patient Paperwork is Completed in 2 Stages

• Prior to your first appointment, our office will e-mail or mail you initial paperwork to be completed and brought to your first appointment.
• After your first appointment you will have a Secure Patient Portal established. Within this portal there will be additional medical history information forms to complete.
